New Patient Intake Forms
To help expedite your first visit, please download, print and complete the forms in the link above. Bring them with you along with your photo ID and (if filing on insurance) your insurance cards. If you choose to fill out the patient form packet at our office, please arrive 15 minutes early to allow enough time to get them completed before your scheduled appointment with the Therapist.
What to expect
Initial Evaluation Appointment:
Expect a full hour, one-on-one consultation with our Doctor of Physical Therapy. See more in our FAQ section.

Standard Appointments:
Expect 45 minutes for standard appointments. Wear clothing you are comfortable and can move in. Your visits may be with our Doctor of Physical Therapy, or our Physical Therapist Assistant, and may slightly overlap with other clients, but we strive to give you as much individual time as possible.

Cancellations:
Because of our goal to give you- and every client- the best possible session, please let us know when you cannot make your scheduled appointment. You can review our No Show/Cancellation Policy which is part of your intake paperwork, at the link below.

Payments:
Please be prepared with payments, if applicable, when you arrive. If you should receive a bill in the mail that is not what was discussed at your first appointment, please contact the Business Operations Manager at office@pineywoodspt.com.
